Join us at East Mountain House for an informative evening exploring Five Wishes, one of the nation’s most widely used advance care planning tools. The presentation will be led by Brenda Fife, CDP, BCPA, a nationally Board Certified Patient Advocate and Certified Dementia Practitioner who serves on the East Mountain House Advisory Board.

Drawing on more than 15 years of experience in senior living, hospice, and patient advocacy, Brenda will guide participants through the Five Wishes document and explain how it helps individuals clearly express their personal, medical, and emotional wishes for care.
This program is designed to help individuals and families begin thoughtful conversations about future healthcare decisions with clarity and confidence.
